COURSE APPROVALS:
RDH Dental Services is an ADA CERP Nationally Recognized Provider. "Continuing education credits awarded for participation in the CE activity may not apply toward license renewal in all states. It is the responsibility of each participant to verify the requirements of his/her state licensing board."

New Jersey, Pennsylvania, Delaware, New York, Virginia State Boards of Dentistry and DANB, Inc. all accept the credits offered by ADA CERP nationally recognized providers toward their requirements. The Maryland Board of Dentistry accepts "clinically relevant" courses offered by an ADA CERP nationally recognized provider.

The Academy of General Dentistry accepts credits from ADA CERP nationally recognized providers toward the FAGD and MAGD awards.

CONFIRMATION LETTER:
A letter will be mailed or e-mailed (along with directions) approximately ten days before the course date.

TUITION DISCOUNTS:
For individuals: When registering for more than one course, deduct $5 from each course fee. Discounts must be taken at the time of initial registration. No combined discounts.
For offices: To qualify, 4 or more attendees must register to be able to deduct $5 from each course fee (for each registration). No combined discounts.

COURSE CE CERTIFICATES:
CE certificates are awarded upon completion of course. There will be a $5 charge for the replacement of each certificate.

RDH Dental Services reserves the right to substitute faculty or to modify the agenda when circumstances are beyond our control. RDH Dental Services reserves the right to change the location of a seminar.

CANCELLATION/REFUND POLICY:
Tuition is non-refundable. Considerable advanced planning is necessary for each course. A request made up to 7 days before the scheduled course will be honored with a voucher to a future course. This voucher, less a $20 fee per registration, is valid for one year from the date of the original course. This fee is necessary to cover nonrefundable materials and facilities costs incurred by us for each participant. A transfer at no cost can be made from one site to another, providing space is available. Call to confirm availability. If unable to attend, another person may attend in your place. If a course cannot be held for reasons beyond the control of RDH Dental Services, the registrant will receive a voucher for a future course.
A charge of $20 will be applied to all returned checks. Non payment of full tuition may result in the notification to the state board of dentistry and cancellation of CE credits.
